Today's hard economic times is affecting nearly everyone in one form or another.
As we all make adjustments in how we live as single individuals or as members of a family, health care costs and dental treatment expenses are commonly controlled by delaying treatments whenever we can and for as long as we can.
In our practice Dr. Sussman and Dr. Pogal started "modularizing" treaments a few years ago so costs could be more easily managed for getting treatments that prevents certain conditions from getting worse while delaying other, less critical treatments.
Bad Bite - Occlusal Disorders on the Rise
Orthodontic issues (young and adult patients), routine extractions (i.e. wisdom teeth), dental bridge replacements, crown replacements and even denture replacements are, for good reason, being delayed.
Extraction dentistry is on the rise, since it is viewed as a reasonable treatment alternative when certain recommended "best choice" treatments are not within easy reach. It is not uncommon, for example, for patients to elect extraction over a Root Canal treatment especially in cases where there may be a risk of retreatment in several years due to normal wear and tear.
In extreme cases, teeth that may need only simple fillings or filling replacements are being left to break off or wear down at a rapid pace.
Miniscule changes that routinely occur in our bite brought about by delaying certain treatments are difficult for many dentists to treat because of the inability to differentially diagnose any "observed" change in our teeth and/or jaw movement - positioning.
Bite: Tap Tap Tap
We are all familiar with the bite (carbon) paper ritual when we get a new filling, crown, bridge etc. For patients who have a normal bite to begin with, this common registration of high and low spots is efficient in most cases.
Patients who have unusual bite characteristics benefit little, if at all, with this type of bite assessment. Everyone has heard the horror stories of a person getting a simple filling only to have adjacent teeth ground down in an effort to relieve pain... that is unsuccessful.
Because accurate bite assessment is difficult for certain types of patients, the "tap tap tap"
procedure is totally ineffective and in some cases, can be destructive.
Diagnosis First: Treatment Plan Development Second
Unusual dentition, odd jaw sizing, malaligned teeth, missing molars, loose or cracked bridgework, canine guidance failure, bone loss (especially for denture wearers) and many other unique conditions require instrumentation that can measure several occlusal and jaw motion events simultaneously.
The technical data that measures biting pressures and jaw muscle movement and tension is converted into numerical and graphical representations of what is occuring, in real time. It is the quality of objective diagnostic data that determines what treatment will work... and what won't.
Limited Availability and Access
General dentists typically do not acquire nor have access to the instrumentation that is routinely used by dentists and oral surgeons who diagnose and treat jaw joint and occlusal dysfunctions. As with complicated Root Canal treatments that require an Endodontic specialist, general dentists don't treat complex tmj or neuromuscular dentistry disorders.
